About HCBS Waiver for the Frail Elderly

The Home and Community Based Services (HCBS) Waiver for the Frail Elderly (FE) provides community-based, in home services to enable the frail and elderly to remain in their own homes and avoid placement in a long-term care facility. The provision of HCBS is based upon the identified needs of the recipient and available funding. ADSD assists recipients with accessing other available services as needed. This program is similar to the COPE Program.

What Services Can HCBS FE Waiver Provide?

Case Management

Assistance by a licensed professional in determining the needs of the client, arranging and monitoring services. Identification of resources and assisting recipients in locating and gaining access to waiver services, as well as needed medical, social, educational, and other services regardless of the funding source.

Homemaker

Assistance with:

• Meal Preparation
• Light Housekeeping
• Laundry
• Shopping

Adult Day Care

Provision of supervision and activities in an Adult Day Care center for 4 or more hours per day on a regularly scheduled basis, for one or more days per week, in an outpatient setting.

Adult Companion

Provides non-medical care, supervision and socialization to a functionally impaired recipient in his or her home or place of residence, which may provide temporary relief for the primary caregiver.

Personal Emergency Response System (PERS)

An electronic device connected to the phone that enables persons at risk to secure help in an emergency for recipients who live alone in a private residence, or who are alone for significant parts of the day in that residence, have no regular caregiver for extended periods of time, and who would otherwise require extensive routine supervision.

Chore Service

Chore services are intermittent in nature and may be authorized as a need arises for the completion of a specific task which other left undone poses a home safety issue.

Respite

Respite care is provided in the recipient's home on a short-term basis because of the absence or need for relief of the primary caregiver.

Augmented Personal Care (provided in residential care settings)

Augmented personal care provided in a licensed residential facility for groups or assisted living facilities is a 24-hour in home service that provides supervision and assistance for functionally impaired elderly recipients with basic self-care and activities of daily living. This care is over and above the mandatory service provision required by regulation for residential facility for groups.

Who is Eligible for the HCBS FE Waiver?

To be eligible for services, an individual must meet the following requirements:

Be 65 years of age or older.

Be at risk of institutionalization (nursing home placement) if services are not provided (meet Level of Care for nursing home placement).

Be financially eligible (call for current income guidelines).

How Can I Apply?

An applicant, community partner or caregiver may fill out the CBC Program Application, and submit to the Aging and Disability Services Division (ADSD) office nearest to them. An applicant may also telephone their local regional office and provide the needed information over the phone to an ADSD intake team member. List of Regional Offices.

Once a completed and signed application is received by ADSD office, an ADSD intake case manager will contact the individual to determine which services and program are most appropriate for them.

Once an applicant is determined eligible for the HCBS FE Waiver, a case manager will remain in close contact with the recipient to assure services are appropriate and are meeting the needs of the recipient.

ADSD personnel are dedicated to the provision of quality services, which assist individuals to remain independent in their own homes.
